COVID-19 GUIDELINES & WAIVER FORM

Skinfinity is temporarily closed due to Governor's orders in response to COVID-19.  We appreciate everyone's understanding and if you have any questions, please reach out!  Please read the below guidelines carefully, these guidelines and Skinfinity's protocols will be adjusted as needed. The below waiver and Consent for treatment must be submitted before your scheduled appointment.  You are welcome to submit your form at any point, even if you do not have a scheduled appointment. All forms will be saved to your file. We look forward to getting back to a new normal!

SKINFINITY'S APPOINTMENT & SERVICE GUIDELINES

-Each service provider will see a maximum of 5 clients per day to allow time for sanitizing between each appointment & to control the number of people who are coming in the facility.

-SCHEDULING: Online booking will be temporarily unavailable, to schedule an appointment please text us at

-All service providers will be wearing a mask at all times & gloves will be worn for the entire duration of each service.

-Please wear a mask at all times inside the facility. If you are scheduled for a facial, you will be allowed to remove your mask once inside the treatment room. You will be required to wear your mask during all other services. 

-Please do not arrive early to your appointment. You may come in at your appointment time, we will be spacing clients enough so you do not need to text when you arrive.  If for any reason we are running behind, we will text you.

-Leave all belongings inside of your car, aside from your wallet & keys, if possible.

-We will be checking your temperature upon arrival, if you have a temperature of 99.0 F or higher, we will have to reschedule your service to another day.

-Gloves and/or hand coverings will not be allowed inside the facility. 

-The restroom door handles, sink knobs, soap dispenser, and toilet seat / flush handle will be disinfected in between every client. However, we suggest you not to use the restroom, aside from washing hands, unless necessary.

-As always, fresh, clean sheets, towels, gowns and head wraps will be used for each facial client. A fresh, clean sheet or table covering will be used all all other service types.  A clean blanket will be provided for facial clients by request only. No blankets will be provided during any other services at this time.

-We will not be using a steamer in facials, your skin will be steamed and softened, for extractions, using a hot towel instead.

-We will be following all state board recommendations & guidelines for proper sanitation, as always. All equipment, tools and surfaces will be sanitized between each appointment. All service providers have completed the BARBICIDE COVID-19 Certification and are following all new recommendations for disinfecting during this time. Additionally, we will also sanitize all doorhandles and areas used, such as the front desk / lobby between each appointment.

-PAYMENT: Electronic payment, via Venmo: @skinfinity, is preferred. We will also accept card, cash & check.

-CANCELLATIONS/NO-SHOWS: We will not be charging a cancellation fee at this time for reasons of illness. We do however ask that you please give us 24 hours notice if you need to cancel, when possible. Our new booking system will request you to confirm your appointment. ALL APPOINTMENTS MUST BE CONFIRMED 24 HOURS PRIOR, IF YOU DO NOT CONFIRM 24 HOURS PRIOR TO YOUR APPOINTMENT, YOUR APPOINTMENT WILL BE CANCELLED. Please understand, we have many clients waiting to see us.

-If you are not feeling well, have any cold or flu-like symptoms, have any symptoms related to COVID-19, please cancel / reschedule your appointment. If your service provider is sick or presents any symptoms of illness at all, you will be notified and rescheduled. If you have been in contact with anyone that has been diagnosed with COVID-19 or has/had symptoms of COVID-19 in the last 14 days and/or the 14 days prior to your scheduled appointment, please cancel or reschedule your appointment.

-We reserve the right to refuse service to anyone, at anytime, prior to and during scheduled treatment.

COVID-19 CONSENT FOR TREATMENT

I have read Skinfinity's Appointment & Service Guidelines. I understand that because aesthetics and services provided at Skinfinity involves maintained touch and close physical proximity over an extended period of time, there may be elevated risk of disease transmission, including COVID-19. By Submitting this form, I acknowledge that I am aware of the risks involved from receiving treatment during this time. I voluntarily agree to assume those risks and I release and hold harmless the practitioner and/or Skinfinity from any claims related thereto. I give my consent to receive treatment by this practitioner at Skinfinity. I am also stating that: I have not had a fever in the 24 hours prior to my appointment of 99.0 degrees Fahrenheit or higher, I do not have or have not recently had any flu-like symptoms - including, sore throat, shortness of breath, difficulty breathing, cough or tightness in chest. By submitting this form, I am stating that I have not been in close contact with anyone in the last 14 days or the 14 days prior to my appointment, who has been diagnosed with COVID-19 or has COVID-19 type symptoms. I myself, have not been diagnosed with COVID-19 and do not have COVID-19 symptoms. If I have been diagnosed with COVID-19 in the past, I have since been re-tested and shown negative of having COVID-19.
